The Assertive Community Treatment (ACT) Program provides intensive behavioral health services for individuals with serious and persistent mental illness, aiming to stabilize their symptoms and enhance community integration.

What is Assertive Community Treatment?
Eligibility criteria for our Assertive Community Treatment (ACT) program include a diagnosis of serious and persistent mental illness, demonstrated difficulty in maintaining community stability, and ineffectiveness of other available services. Operated by an interdisciplinary team available 24/7, ACT serves adults aged 18 and older, particularly those with complex treatment needs and limited success in traditional community placements.
Referrals come from various sources, including hospitals and mental health centers. The program offers ongoing outreach and treatment. Assessment occurs within 30 days of entry, covering psychiatric history, social functioning, substance use, and more. ACT operates primarily in the community, with strict medication procedures and crisis intervention protocols.
Overall, ACT prioritizes personalized support to facilitate stability and meaningful community living for individuals with serious and persistent mental illness.
