Heritage Services

The Williams Program 

The Williams Program is a grant-based program with guidelines provident by IDHS and UIC College of Nursing through the Williams Consent Decree.

 

The goal of this program is to assist individuals residing in Decatur Manor (Specialist Mental Health Rehabilitation Facility) with transitioning to the community with appropriate support.

How does it work?

The Williams team provides outreach, assessment, transition services, oversight, and care management during the entire transition process including ensuring adequate supports are in place post-transition for the client to safely and successfully remain in the community.

 

The current contract requires the Williams Program to transition 20 separate individuals out of Decatur Manor each fiscal year. Williams Program participants are determined based on Medicaid-eligibility and admission to Decatur Manor. Additional referrals are not accepted.
